Ford Explorer / Firestone Tire Tread-Separation Rollovers

Vision

What was assumed or expected to hold, before the failure.

Firestone tires fitted as original equipment on the Ford Explorer were assumed roadworthy at Ford's recommended inflation pressure, and the vehicle-tire combination was assumed stable in an emergency maneuver.

Timeline

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. Tread-belt adhesion issues in the Firestone ATX and Wilderness AT tires were worsened by the lower tire pressure Ford recommended for Explorer ride and stability.
  2. Tread separation at highway speed, more common in hot climates, combined with the Explorer's higher center of gravity to produce rollovers.
  3. Early warning data, including a 1998 insurance-claims researcher's findings, were not acted on quickly by NHTSA or the manufacturers.
  4. NHTSA opened a formal investigation in May 2000 after complaints reached the dozens.
  5. Bridgestone/Firestone recalled about 6.5 million tires on 9 August 2000.
