Great Heck (Selby) Rail Crash

Vision

What was assumed or expected to hold, before the failure.

The motorway embankment barrier above the railway line was assumed sufficient to prevent a road vehicle from leaving the carriageway and reaching the tracks below, and drivers were assumed to self-regulate against driving while dangerously fatigued.

Timeline

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. The driver, Gary Hart, stayed up all night on a phone call before driving, resulting in severe fatigue.
  2. He fell asleep at the wheel of his Land Rover, which was towing a trailer, on the M62 motorway.
  3. The vehicle left the carriageway and went down the embankment onto the East Coast Main Line.
  4. A GNER passenger express struck the Land Rover, partially derailing, and then collided head-on with an oncoming freight train.

ROI: the outcome

10 people died, including both train drivers, and 82 were injured -- the deadliest UK rail disaster of the 21st century. Gary Hart was convicted of causing death by dangerous driving. The Health and Safety Executive's investigation prompted recommendations on rail-vehicle crashworthiness and embankment barriers.
