Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. The perpetrator's controlling behavior prior to separation was not formally recognized or risk-scored as escalating toward lethal violence.
  2. The inquest found a specific failure to recognize the risk of intimate partner homicide that results from separation in a coercive-controlling relationship.
  3. Despite finding individual police officers acted appropriately in isolated contacts, the inquest identified missed opportunities for earlier systemic intervention.

The inquest's findings directly contributed to Queensland passing legislation criminalizing coercive control, in force from 26 May 2025, colloquially known as Hannah's Law.
