Hatfield Rail Crash

Vision

What was assumed or expected to hold, before the failure.

Rail track was assumed to be maintained on inspection schedules that would catch developing metal fatigue before it became critical, under an outsourced maintenance contracting model.

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. A section of rail on the East Coast Main Line had developed extensive rolling contact fatigue from repeated wheel loading.
  2. The condition had been identified months earlier, but replacement rail, though ordered, had not yet been installed, and the known-defective track was not adequately speed-restricted in the interim.
  3. The fragmented, privatized maintenance contracting structure created ambiguity and delay in who was responsible for acting urgently.
  4. The rail shattered into over 300 pieces under a passing high-speed train.
  5. The train derailed at high speed.

ROI: the outcome

4 people died and over 70 were injured. The crash triggered a nationwide emergency review and mass replacement of similarly worn rail, and was a central factor in the replacement of Railtrack by Network Rail in 2002.
