Vision

What was assumed or expected to hold, before the failure.

A prior repair to the aircraft's aft pressure bulkhead, performed by the manufacturer years earlier, was assumed to restore full structural integrity to original design specification.

Timeline

  1. 1978The aircraft suffers a tailstrike and receives the flawed repair.
  2. 1985The bulkhead fails in flight and the aircraft crashes into Mount Takamagahara.

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. Seven years before the accident, the aircraft suffered a tailstrike, damaging the rear pressure bulkhead.
  2. The repair used a single row of rivets to join a patch plate rather than the required double row, roughly halving the joint's fatigue strength.
  3. Over thousands of subsequent pressurization cycles, metal fatigue cracks grew undetected along the improperly repaired joint.
  4. The bulkhead ruptured explosively in flight, and the resulting decompression blew off part of the vertical stabilizer and severed all four hydraulic lines.
  5. With no hydraulic flight controls, the crew fought for over 30 minutes using only engine thrust before the aircraft crashed into a mountainside.

ROI: the outcome

520 people died, the deadliest single-aircraft accident in aviation history. Led to major revisions in repair-approval oversight and renewed discussion of hydraulic system redundancy across the industry.
