Lac-Megantic Rail Disaster

Vision

What was assumed or expected to hold, before the failure.

A single-person train crew, combined with hand brakes plus one locked, unattended air-braked and idling locomotive, was assumed sufficient to safely secure a parked train carrying volatile crude oil overnight on a mainline grade.

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. The railway had, as a cost-cutting measure, moved to single-person crews on long unattended stops.
  2. The train, carrying tankcars of crude oil that was misclassified as lower-volatility than it actually was, was parked overnight on a mainline grade with only hand brakes set and the lead locomotive left running to power the air brakes.
  3. A fire broke out in the idling locomotive; responding firefighters shut it down per protocol, which caused the air brakes to bleed off over time without anyone realizing this compromised the train's securement.
  4. The unattended train began rolling downhill, gathering speed over roughly 7 miles, and derailed at high speed entering the town.
  5. The ruptured tankcars, of an older design known to be prone to puncture, ignited in a series of explosions in the town center.

ROI: the outcome

47 people died. The disaster led to new North American regulations phasing out older tank-car designs for flammable liquids, mandatory minimum crew rules, and stricter crude-by-rail classification requirements.
