Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. Wayne Couzens had a documented history of alleged sexual offending, including at least eight reported indecent-exposure incidents, dating back nearly two decades before he murdered Sarah Everard.
  2. These reports were not acted upon by the police forces that received them.
  3. Misogynistic and abusive language and behavior by Couzens among colleagues went unchallenged internally.
  4. Vetting failures across three different police forces meant Couzens was repeatedly cleared for recruitment and continued service despite red flags.
  5. In March 2021, while a serving Metropolitan Police officer, Couzens used his police warrant card and handcuffs to falsely arrest, then abduct, rape and murder Sarah Everard.

Lady Elish Angiolini's inquiry (Part 1 report, published 29 February 2024) concluded Couzens should never have been a police officer and made 16 recommendations on vetting, misconduct reporting and officer accountability.
