  1. The initial investigation into the racially motivated murder of Stephen Lawrence was marked by delayed arrests and poor handling of early evidence.
  2. The inquiry found professional incompetence, institutional racism and a failure of leadership by senior officers.
  3. The inquiry defined institutional racism as a collective organizational failure detectable in processes, attitudes and unwitting prejudice.
  4. The inquiry found this extended beyond the Metropolitan Police to policing generally.
