What was assumed or expected to hold, before the failure.

Standard radio phraseology and air traffic control procedures were assumed sufficient to safely sequence aircraft movements on a single runway, even under abnormal, congested, fog-degraded conditions.

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. A bomb threat at a nearby airport diverted multiple large aircraft to the smaller Los Rodeos airport, creating unusual congestion, with aircraft parked on the only taxiway.
  2. Dense fog rolled in, severely limiting visibility on the runway.
  3. One aircraft, cleared only to taxi to position and await further clearance, began its takeoff roll after the captain misinterpreted an ambiguous radio exchange as takeoff clearance.
  4. A warning transmission from the other aircraft, still taxiing on the same runway, was garbled and not clearly heard.
  5. Unable to see the other aircraft until the last seconds due to fog, the taking-off aircraft struck it during its takeoff roll.
