Vision

What was assumed or expected to hold, before the failure.

The center wing fuel tank, even when nearly empty and thus containing flammable vapor rather than liquid fuel, was assumed to pose negligible explosion risk absent an external ignition source.

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. The aircraft's center wing fuel tank was nearly empty prior to departure, meaning it was filled largely with flammable fuel-air vapor rather than liquid fuel.
  2. The center tank sat adjacent to air-conditioning packs that ran for an extended period on the ground before departure, heating the tank and its vapor to within the flammable range.
  3. Aging wiring in the 25-year-old aircraft ran near the fuel quantity indication system wiring that penetrated the tank, and a short circuit allowed excess voltage into that wiring.
  4. That short circuit created a spark inside the tank.
  5. The spark ignited the flammable vapor inside the center wing tank, and the resulting explosion broke the aircraft apart in flight.

ROI: the outcome

230 people died. After ruling out other causes, the NTSB drove sweeping FAA rule changes requiring fuel tank inert-gas systems on new and retrofitted aircraft, plus tightened wiring-inspection and wire-separation standards fleet-wide.
