Unequal Treatment (Institute of Medicine Report)

Clinical diagnostic and treatment decisions were assumed to be applied consistently regardless of a patient's race or ethnicity, once access and socioeconomic factors were controlled for.

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. Congress mandated the study following an earlier federal report on Black and minority health.
  2. The committee reviewed hundreds of studies and found racial and ethnic disparities in treatment quality persisted even after controlling for access, insurance status, and clinical presentation.
  3. The report attributed part of this gap to clinician-level bias, stereotyping, and differential communication in the clinical encounter, not only structural access issues.

ROI: the outcome

The report became the foundational US reference document for research on race-based clinical bias, prompting federal and academic-medical-center initiatives on implicit-bias training and disparities monitoring, followed two decades later by a National Academies review of progress.
