Vision

What was assumed or expected to hold, before the failure.

A fan disk manufactured to aerospace-grade titanium standards, subject to routine ultrasonic inspection, was assumed unlikely to fail catastrophically in flight, and triple-redundant hydraulic systems were assumed unable to all be lost simultaneously.

Lens: the chain

The actual sequence of decisions and conditions that produced the outcome, as documented.

  1. The tail-mounted engine's fan disk had a manufacturing-origin metallurgical defect that had gone undetected through years of inspections, below the resolution of the ultrasonic method used at the time.
  2. The defect grew via fatigue cracking over thousands of flight cycles.
  3. The fan disk disintegrated in flight, and shrapnel severed all three hydraulic lines, an outcome considered virtually impossible given the systems' assumed separation.
  4. With no hydraulic flight controls, the crew improvised control using only differential engine thrust on the two remaining engines.
  5. The crew managed a partially controlled crash landing, but the aircraft broke apart on touchdown.

ROI: the outcome

111 people died and 185 survived, a widely noted survival rate given the total loss of flight controls. Led to redesigned fan disk inspection techniques and renewed focus on hydraulic system routing to prevent common-mode loss.
