When something goes wrong, attention turns to what happened: the sequence of events, the actors involved, the outcome. That part is usually reconstructable.

What is harder- sometimes impossible- is recovering the reasoning that produced the decision. The context at the time, the information that was available, the alternatives considered and rejected, the assumptions that seemed sound. Not because anyone chose to conceal it, but because no one decided it needed to be preserved.
